1. I am afflicted & diseased with the Stip Complaint or disease in the left hip or thigh. Has been p-- for 30 years a piece of the bone come out from the hip joint which was pr-- for a big time it was a scale or shell scalloped or forked
Disqualified the permit from big endurance or labor

2. Hemorrhage  of lungs or spitting of blood

3. Ruptured in right side about 15 years ago. Do not wear a truss but I use extreme caution & guard against its escape or protusion.

1. Injury to ancle joint and fracture of one of the bones above said joint of the left leg

2. Great constipation of bowels requiring medicines or pills very frequently

3. Predisposed to inflamation of the bowels - Have had three attacks of same during the last year

     I can do light work but heavy work is done by hired hands as my health and physical strength is not adequate to the performance of heavy labor

4. Weakness of the eyes

CLAIM OF EXEMPTION.

I Mackey Taylor residing in the town of Broome County of Schoharie, claim exemption from Military Duty, for the following reasons, to wit:
Several debility & am not able to perform active exercise for more than two years. have pain in my lefty side Cannot walk any great distance without great difficulty with my feet becoming sore & fatigued

Mackey Taylor being duly sworn, doth depose and say that he is the identical person named in the foregoing statement of exemption, and that the matters therein stated, upon which his claim for exemption from Military Duty is founded, are true.
