On Ancestry.com, Pennsylvania Prison, Reformatory, and Workhouse Records 1829-1971, it shows that he went to prison on 13 Dec 1910. He had a three-year sentence for false pretense and embezzlement. His occupation is listed as banker; perhaps this was the end of his banking career.


The name on his death certificate is John Raymond Pacovsky. Cause of death was chronic myocardialfibrosis and chronic nephritis; chronic alcoholic listed as contributory. Place of death was General Hospital. His place of birth looks like Vicez, Czechoslovakia. Not sure if that's right, I couldn't find anyplace with that name. No father's name, mother's name listed as Barbara Pacovsky. His wife's name is listed as Gertrude Pacovsky, so I guess he never remarried.
