The said pensioner, Thomas Stevens, served in the militia of the state of Kentucky in the 10th Regiment under the command of Colonel William E Boswell. This appears from the certificate of Captain William Sebree who also stated that the said Thomas Stevens received a wound from a ball on May 5, 1813, while in the service of the United States of America. It was stated that the bullet entered the left side of the lower jawbone in three different places, carrying away several teeth and fracturing others. And it was stated that therefore the said Thomas Stevens was practically disabled. September 7, 1822

It was also stated that the said pensioner was detached into service on March 29, 1813 and was wounded in a sortie at Fort Meigs in the state of Michigan or Ohio while the regiment under the command of Colonel John Miller. He was paid off in the month of August 1814. The said pensioner was honorably discharged in the month of September 1813, according to the muster rolls.

On September 27, 1822 in Campbell County Kentucky, the affidavit of Coleman Rogers, MD was also given. The said surgeon stated and swore that the said Thomas Stevens had served in the capacity of a private soldier in the 10th Regiment of the Militia of the State of Kentucky, under the command of Colonel William E Boswell and in the company under the command of Captain William Sebree. The said doctor stated and swore that the said applicant had been wounded May 5, 1813 and that due to the said wound, he was at least on-half disabled.

On October 8, 1822 in Campbell County Kentucky the affidavit of Hugh Rackford was also given. The said deponent was the surgeon of the 48th Regiment of Kentucky Militia. The said deponent stated and swore that he had carefully examined the wound of the said Thomas Stephens and the surgeon declared it to be his opinion that the soldier was at least one-half disabled by the said wound he had received in the service of the United States of America.

A certificate dated February 27, 1823 from the Third Auditor’s Office of the Treasury Department stated that the said Thomas Stevens had been honorably discharged in the month of September 1813 after having served his country for a tour of six months duration.

On July 17, 1827, the said pensioner, Thomas Stevens, appeared in open court and stated that his pension had been reduced below the rate of $4 per month, and he applied for an increase to the year 1836. On July 17, 1827 the affidavit of J Bennett was also given in Campbell County Kentucky. The said deponent was the acting surgeon of the United States Garrison at Newport in Kentucky and the said doctor stated and swore that the said pensioner was really wounded. And he was of the opinion that the said Thomas Stevens deserved the pension amounting to $4 per month.

On January 27, 1833 in Scott County Kentucky, the affidavit of William Sebree was also given. The said deponent was a late Captain in the 10th Regiment of Militia of the state of Kentucky. He swore that he certified the said Thomas Stevens had served in the War of the Revolution in the capacity of a private soldier. And that he had been wounded at Fort Meigs while serving in the regiment under the command of Colonel John Miller. The said Captain Sebree further added that Thomas Stevens had entered the service on March 29, 1813 and that he was a good soldier. And that he had been honorably discharged from the service of the United States while it was at war with the King of Great Britain.

At the Northern Bank of the State of Kentucky, on September 6, 1836, the said pensioner, Thomas Stevens, appeared for the purpose of asking that he be transferred to the roll of Cincinnati in the state of Ohio, as he then resided a few miles from there. The certificate of pension was dated January 7, 1823, by which he received $4 per month. The amount of his disability was given. This was increased on July 17, 1837.

In the month of June 1844, the said Thomas Stevens again applied for an increase of pension. This was #983. He was discharged by a certificate of January 27, 1823, March 5, 1823, Acts of April 24, 1816 and on February 4, 1820.

The said Thomas Stevens was on the Kentucky Roll of Pensions at the rate of $4 per month to commence on January 23, 1823 and his certificate of pension for that amount was issued March 5, 1823 and it was sent to Honorable J T Johnson, House of Representatives
