	This form is NOT in the standard format as provided by the National Board of Certified Genealogists. It should be used ONLY as a single family group sheet with additional spouses entered on a separate form. Certain blanks MUST be entered with information before the completed form can be processed by the FGS Manager. Person's unable to provide required information should contact the FGS Manager direct for assistance. This form is automatically sent to the FGS Manager when submitted.

In order to process your data file for the archives, the following form fields MUST be filled in correctly.
1. Permission to publish the data on the Bullitt County KYGW Project web site
2. Type of FGS file to be processed - additional or corrected data should be noted in comment sections
3. Family surname in all capital letters (spaces allowed when applicable)
4. Submitter's name or the person sending information to the FGS Manager
5. Submitter's email address as a contact for reference - Updated email addresses are posted only when notification is given
NOTE: If *name* or *email* fields are submitted blank, it will be assumed privacy is needed and the FGS Manager will become the submitter of the file data to be published with permission granted.
