Mrs. Otha Shaw of Whitestown, IN< made application for a military headstone for Roy V. Shaw, born 13 Jul 1887, died 30 Apr 1948. To be buried Mts. Run cemetery, Whitestown. He enlisted 29 Mar 1918 and was discharged 29 May 1919. Private in Company F, 104th Ammunition Train, 29th Division. (Penciled in red on back of the application is - "Dishonorable Discharge" - but the stone was allowed and ordered 12 Oct 1948.

Lebanon, Ind. - Services for Otha Shaw, 82, Lebanon will be held at 1:30 p.m. Monday in Russell & Hitch Funeral Home, where friends may call after 7 p.m. Saturday. She died Thursday in Witham Hospital. A native of Boone County, she had been employed at Whitestown Telephone Co. She was the widow of Roy V. Shaw. Survivors include three daughters, Mary E. Highland, Emma M. Magoon and Carol L. Jones, and a son, Claudius Shaw.
