DECLARATION FOR WIDOW'S PENSION

Act of May 1, 1920

State of North Dakota, County of Aurora, ss:

On this 12 day of June, 1921, personally appeared before me, a Notary Public within and for the County and State aforesaid, Barbara Hepner, who, being duly sworn by me according to law, declares that she is [blank] years of age and that she was born December 20, 1839, at near Bridgeport, Wayne Co. Ohio.

That she is the widow of William T. Hepner, who enlisted Sept 11, 1861, at Knox Indiana, under the name of William T. Hepner, as a Corporal in Captian ["in" lined out] Joseph P. Collins Co. D 29th Regiment Indiana Volunteers and was honorably discharged July 29, 1862, having served ninety days or more, or was discharged for or died in service of a disability incurred in the service and in the line of duty, during the CIVIL WAR.

That he also served [blank] (Here give a complete statement of all other military, naval, or coast guard service, if any, at whatever time rendered.)

That she was married to said soldier ['(or sailor)" lined out] April 19th, 1857, under the name of Barbara Cripliver, at Tipton ["Indian" lined out] Iowa by Rev. Geo. Miller; that she had not been previously married; that he had not been previously married and that neither she nor said soldier (or sailor) was ever married otherwise than as stated above.

That said soldier (or sailor) died April 2nd, 1921, at Soldiers Home[,] Sawtelle, Calif.; that she was not divorced from him; and that she has not remarried since his death.

That the following are the ONLY children of the soldier (or sailor) under sixteen years of age, who are now living, namely:

That the above-named child[blank] of the soldier (or sailor) {is/are} [blank] now receiving a pension, and that such child[blank] {is a/are} member{blank] of her family and [blank] cared for by her.

That she has not heretofore applied for pension, the number of her former claim being [blank]; that said soldier ["(or sailor)" lined out] was [blank] a pensioner, the number of his pension certificate being 786094.

That she makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the ACT OF MAY 1, 1920.

Subscribed and sworn to before me this 11 day of June, 1921, and I hereby certify that the contents of the above declaration were fully made known and explained to the applicant before swearing, including the words in, sailor, Indiana, Code Gingrich erased, and the words none, added; and that I have no interest, direct or indirect, in the prosecution of this claim.
