William T. Hepner, Certificate of Disability for Discharge
ARMY OF THE UNITED STATES.
CERTIFICATE OF DISABILITY FOR DISCHARGE.
(To be used, in duplicate, in all cases of discharge on account of disability.)
William T. Hepner a Corporal of Captain Joseph P. Collins Company, (D) of the Twenty Ninth Regiment of ["United States" lined out] Indiana Volunteers was enlisted by Capt. Joseph P. Collins of the (29") Regiment of Indiana Volunteers Knox Stark Co. Indiana on the Eleventh day of September 1861, to serve (3) years; he was born in Warren in the State of Ohio is Twenty Seven years of age, Five feet ["Nine" appears to have been written over "Six"] inches high, Light complexion, Brown eyes, Brown hair, and by occupation when enlisted a Carpenter. During the last two months said soldier has been unfit for duty 60 days.
By reason of long continued disease of the Lungs
Station: Camp near Stevenson Ala.

I CERTIFY, that I have carefully examined the said [blank] of Captain [blank] Company, and find him incapable of performing the duties of a soldier because of Debility from disease of the Lungs. I believe his disease will ever render him unfit for a soldier[.] I should consider his disease Phitisis Pulmonalis
