GENERAL AFFIDAVIT.

State of Indiana, County of La Porte, ss:

In the matter of Pension Invalid Claim Certificate No. 362 802 of Co. D., Co. 29th Reg't Ind Volunteers.

Personally came before me, a [blank] in and for the aforesaid County and State, Lorenzo S. Keene, aged 79 years, a citizen of La Porte, no. 301, State Street, County of La Porte, State of Ind., well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to the aforesaid case, as follows:

I was Surgion of the 29th Reg, Indiana Volunteers and was well acquainted with Applicant from the time he enlisted in the 29th Reg. Ind Vol, That at the time of his elistment he was a healthy man & was a faithful soldier while in the service[.] That I often gave him treatment for diarrhea and other ailments more particular while in Camp Nevin Ky. And while in Camp near Corrinth Tenn. At the latter place he became so reduced in health from the diarrhea & hardships of the service he was sent to hospital from which I learned he was discharged.

I further declares that I have no interest in said case, and am not concerned in its prosecution.

Sworn to and subscribed before me this day by the above-named affiant, and I certify that I read said affidavit to said affiant, including the words ---- erased and the words ---- added, and acquainted him with its contents before he executed the same. I further certify that I am in nowise interested in said case, nor am I concerned in its prosecution; and that said affiant is well known to me and is a credible and reputable person for truth &c.
