GENERAL AFFIDAVIT.

State of Ind., County of Starke, ss:

In the matter of Pension Invalid Claim Certificate No. 362.802. of Matthias T. Hepner, Co. "D", Reg't 29" Volunteers.

Persronally came before me, a Clerk Cir Court in and for the adoresaid County and State, Matthew C. Boner, aged Sixty years, a citizen of Knox, No. [blank] Street, County of Stark State of Indiana, well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to the aforesaid case, as follows:

I am a physician and Surgeon of thirty two years experience, and have been acquainted with Mathias. T. Hepner. about twenty or twenty one years, and his family physician nineteen or twenty years.

I have been called to treat him several times, during this during this time. I treated him during an attack of typhoid fever, in April & May 1884, and at other times for Malaria, Rheumatism, Piles, (Hemorrhoids), Hemorrhage from bowles, and in connection with the above named diseases, for Chronic diarrhea, But have not treated him for Chronic diarrhea independent of other diseases. He has been Subject to frequent of Chronic Diarrhea, ever since my first acquaintance with him, which has been the primary of Piles, (Hemorrhoids,) and frequently Hemorrhage from the bowles, for which I have been counciled at different times. For instance; on Page 64: Ledger A. Sept 30" 1895. To prescription for severe attack of Piles with Hemorrhage, btot on by severe attack of Chronic Diarrhea. I have prescribed for him for same trouble a number of times since 1888 and 1890, but on account of the prescriptions being paid for when made, were not charged, and on that account could not give dates.

The reason that I cannot give dates for visits & prescriptions that I have made in this case from the time I located here in 1876 or 1877 until 1888 or 1890, is because I traded my unsettled book accounts to my Brotherinlaw in a land trade. On two or three occasions I have made an effort to

find them to use for Statistical purposes, but did not succeed, they being lost or distroyed.

I further declares that I no interest in said case, and am not concerned in its prosecution.

Sworn to and subscribed before me this day by the above-named affiant, and I certify that I read said affidavit to said affiant, including the words [blank] erased and the words [blank] added, and acquainted him with its contents before He executed the same. I further certify that I am in nowise interested in said case, nor am I concerned in its prosecution; and that said affiant is Personally Known to me, and is a credible person, and his general reputation for truth is good.
