To all whom it may concern.

Know ye, that Matthias T. Hepner a 1st Lieut ["of Captain" line out] Company, (H,) 151st Regiment of Indiana Infty VOLUNTEERS, who was enrolled on the First day of May one thousand eight hundred and Sixty-five to serve One year ["s during the war" lined out], is hereby DISCHARGED from the service of the United States this Nineteenth day of September, 1865, at Nashville Tennessee by reason of S. O. No. 68 Dept. of Tenn in persuance with G. O. No. 94 W D Series of 1865.

(No objection to his being re-enlisted is known to exist*)

Said Matthias T. Hepner was born in Marion in the State of Indiana, is Thirty years of age, [blank] feet [blank] inches high, [blank] complexion, [blank] eyes, [blank] hair, and by occupation, when enrolled, a [blank].
