Sir:
You are respectfully requested to furnish official evidence of the enrollment, muster, service, duty, and discharge ["or death" lined out] of Mathias Hepner, who is alleged to have ["been" lined out] volunteered at Salem, N. J., on the [blank] day of [blank], 18[blank], in Captain Pierce's Company, of Col. Winfield Scott Regiment of 3rd Artillery, and to have been discharged (["or died" lined out]) at [blank] on the [blank] day of [blank], 18[blank].
If the above name is not found on the rolls of said Company, will you so state, and report as to enrollment, &c., in the case of any man bearing a similar name, whom you have good reason for believing to be the soldier inquired for.
