Henry A. Bastin, Declaration for the Rating of an Invalid Pension

 

Declaration for the ["Increase" lined out; "the rating" inserted] of an Invalid Pension.

TAKE NOTICE.- If the declaration is executed before a Justice of the Peace or a Notary Public, the certificate of the CLERK OF THE COURT, as to the official character and genuineness of the signature of such officer must be attached. Neglect to comply with this requirement will cause trouble and DELAY.

State of Indiana, County of Monroe, ss:

ON THIS 22 day of May A. D. one thousand eight hundred and eighty 8 personally appeared before me, a Clerk of the Court within and for the County and State aforesaid, Elizabeth Bastin aged 54 years, a resident of Ellettsville County of Monroe State of Indiana, who, being duly sworn according to law, declares that she is a pensioner of the United States enrolled at the Indianapolis Pension Agency at the rate of $12 and $2 additional for each minor child under 16 years old dollars per month, Certificate No. 228024; by reason of ["disability from" lined out] Being the legal widow of the late Henry A Bastin, dec who died Aug. 26, 1873 from Rheumatism and results incurred in the military service of the United States, while serving as a Private Co. H. 33. Ind. vols.

That she believes herself to be entitled to a ["n increase" lined out] [insert] Re-Rating of her husbands pension or rather a completion of it [end insert] of pension on account of his being discharge by reason of Rheumatism being tottaly disabled when discharged on account of said Rheumatism.

That said soldier made application for pension about 1866 or 1867 on acount of said Rheumatism but no pension was ever paid him. That he died in Aug. 1873. And that after his death his said widow made application & pension was allowed her back to date of her said husband's death when in fact she ought to have received pension from date of her said husband's discharge. She now asks pay from soldier's discharge

That he hereby appoints, with full power of substitution and revocation, James F. Morgan of Bloomongton his true and lawful attorney, to prosecute his claim.
