Henry A. Bastin, Certificate of Disability for Discharge, 1862

CERTIFICATE OF DISABILITY FOR DISCHARGE.

(To be used, in duplicate, in all cases of discharge on account of disability.)

Henry Bastin Private, of Captain James E. Burtons Company, (H,) of the thirty third Regiment of United States Volunteers, was enlisted by Jas. E. Burton, of the 33rd Regiment of Ind Vols, at Martinsville Indiana on the second day of Septr, 1861, to serve three years; he was born in Putnam County in the State of Indiana, is thirty years of age, six feet - inches high, Light complexion, Blue eyes, Light hair, and by occupation when enlisted a Farmer. During the last two months said soldier has been unfit for duty 60 days. (Here consult directions on Form 12, p. 269, Medical Dept. Gen. Reg.) in consequence of a Rheumatic affection of which he has had repeated attacks from slight exposure since he entered the service. With the last attack his limbs have swollen especially the left leg. Has been in Hospital since the first of Jany, 1862

I CERTIFY, that I have carefully examined the said Henry Bastin Private of Captain Burtons Company, and find him incapable of performing the duties of a soldier because of (Here consult par. 1134, p. 215, and directions on Form 12, p. 269, Med. Dept. Gen, Reg.) Rheumatism in the back and legs both of which are atrophied especially the left. He has had several attacks since he entered the service. The last attack has confined him in Hospital since the first of Jany 1862, and has been treated with but very partial relief. He cannot be fit for service again for months to come and probably never.
