NEIGHBORS' AFFIDAVIT.

For the testimony of EMPLOYERS or NEAR NEIGHBORS of soldier (other than relatives), showing his physical disability, since his discharge.

State of Iowa, County of Cedar, ss:

IN THE MATTER of the application for pension of David S. Hepner of Tipton Iowa

ON THIS 11 day of December, A. D. 1896, personally appeared before me, a Walter Swartzbender in and for the aforesaid County, duly authorized to administer oaths, [blank: the prior name is supposed to be in this space, not the one in which it appears] aged 33 years, a resident ["of Thirty Three" lined out and "Tipton" inserted above: "of" inappropriately lined out; "Thirty Three" originally was written here in the wrong space] in the County of Cedar and State of Iowa whose Post Office address is Tipton Iowa and [blank] aged 33 years, a resident of Tipton in the County of Cedar and State of Iowa whose Post Office address is Tipton Iowa [this section for a second affiant was not supposed to have been filled out] well known to me to be respectable and entitled to credit, and who being duly sworn, declare in relation to the aforesaid case as follows: That I have been well and personally acquainted with David S Hepner for Ten years, and [blank] years respectively, and that I have lived in Same neighborhood with David S. Hepner & with in Two miles of his residence during the time.

That I have seen him once every week during that time and have Known him to Complain of Chronic Diahroera almost every time I have conversed [first "with" lined out] with said David S Hepner. And that I ["have" lined out] during my acquaintance with him I have Know him not to do manual labor of any account. I am not able to say what portion of a full days work he can do.

This is to say I have written this with out aid or prompting by any one.

Note.-The witnesses, if not themselves equal to the task of drawing the affidavits, should go to some Notary Public, Justice of the Peace, or other officer or competent person, and have the blank filled out and properly executed.

Sworn and subscribed before me this day by the above-mentioned affiant, and I certify that I read said affidavit to said affiant, including the words [blank] erased, and the words [blank] added and acquainted him with its contents before he executed the same. I further certify that I am in nowise interested in said case, nor am I concerned in its prosecution; and that said affiant is personally known to me and that he is a credible person.

Note.-This can be executed before any officer authorized to administer oaths for general purposes. If such officer uses a seal, certificate of Clerk of Court is not necessary. If no seal is used, then such certificate must be attached.
