GENERAL AFFIDAVIT.

State of Iowa, County of Cedar, ss:

IN THE MATTER OF [blank]

ON THIS 2d day of Oct, A. D. 1894, personally appeared before me, a County Auditor in and for the aforesaid County, duly authorized to administer oaths, J. F. Sanford aged [blank] years, a resident of Tipton in the County of Cedar and State of Iowa whose Post Office address is [blank] and [section for information relating to a second affiant is blank] well known to me to be reputable and entitled to credit, and who being duly sworn, declare in relation to the aforesaid case as follows:

This is to Certify that David S. Hepner has Fistula in Ano, hemorrhoids, Diarrhoea & weak eyes, and to my Knowledge has been so ever since he left the Army, his eyes are growing worse every day, as to manual labor he cannot do one third 1/3 of a mans labor, I Know this man well and have every since he left the Army. I Know that his condition is not the result of vicious habits.

This was written and prepared in his presence from his oral statement, and was not prompted by anyone as set forth above.

I further declare that I no interest in said case and am not concerned in its prosecution.
