GENERAL AFFIDAVIT.

State of Iowa, County of Cedar, ss:

IN THE MATTER OF [blank]

ON THIS 6th day of Oct, A. D. 1894, personally appeared before me, a County Auditor in and for the aforesaid County, duly authorized to administer oaths, R A Peters aged 31 years, a resident of Tipton in the County of Cedar and State of Iowa whose Post Office address is Tipton Iowa and [section for information regarding second affiant is blank] well known to me to be reputable and entitled to credit, and who being duly sworn, declare in relation to the aforesaid case as follows:

Note:-Affiants should state how they gain knowledge of the facts to which they testify.

This is to Certify that David S. Hepner has been afflicted with Fistula in Ano, Hemorrhoids, diarrhoea and weak eyes, He has to my Knowledge been in this condition for several yrs. His eyes are not as strong as they were two yrs. ago and as to manual labor he cannot do one fourth of a mans labor. I have Known this man for ten yrs. and as to vicious habits I haven't any Knowledge of such, he is a worthy man and deserves recognition.

I further declare that I no interest in said case and am not concerned in its prosecution.
