["GUARDIAN'S" lined out] DECLARATION FOR ["MINOR" lined out] CHILDREN'S ARMY PENSION.

STATE OF Indiana

COUNTY OF Morgan } ss:

On this 29 day of May feb A. D., one thousand eight hundred and ninty Six personally appeared before me clerk of the ct. ct. a [blank] within and for the County and State aforesaid, Mary "A". Bolin a resident of Paragon in the County of Morgan and State of Indiana, aged 42 years, who being first duly sworn according to law, doth, on oath, make the following declaration, as ["guardian of" lined out] the ["minor" lined out] child [blank] of Chistopher A. Bolin deceased, in order to obtain the benefit of the provisions made by the Act of Congress, approved ["July 14, 1862" lined out] June 27 1890, granting pensions to minor children, under sixteen years of age, of deceased officers and soldiers. That she is ["the guardian" lined out] an heir of said Chistopher C. Bolin deceased (Co. B 67th Ind) whose father was a Private in Company "B" commanded by Captain Saml Denny in the 67th Regiment of Indiana Vols in the war of 1861; who while in the service aforesaid, and in the line of his duty contracted disease from which he died at St. Louis in the State of Mo on the [blank] day of [blank] 18[blank].

That the mother of the child [blank] aforesaid [blank] on the [blank] day of [blank], and that the date of the birth of his Ward, and [blank] residence [blank] as follows, to wit:

[blank] born on the [blank] day of [blank] 16[blank]

That said Mary A. drew a pension as minor of decedent to Jay. 24th 1871 when She became Sixteen years old. She lost her vision when Seven years old and has been absolutely blind ever Since. She is in every Sinse of the word helpless. She therefore applies to the Com of Pensions to be placed on the rolls as pensioner under provision of June June 27th 1890.

He further declares ["that the" lined out] parents of his ward were married at [blank] in the ["County of" lined out], ["State of" lined out] on the [blank] day of [blank] in the year 18[blank], by [blank].

He also declares that neither he nor his ward ha[blank] in any manner been engaged in, or aided or abetted the rebellion in the United States. He hereby appoints J. J. Johnson of Martinsville, County of Morgan, State of Indiana his true and lawful Attorney, with full power of substitution, to prosecute this claim for pension, and receive the certificate to be issued therefor.

Northen residents of [blank] County of Morgan, State of Indiana, whom I certify to be respectable and entitled to credit, and who being by me first duly sworn, say that they were present and saw [blank] sign ["his" lined out] her name to the foregoing declaration and Power of Attorney; and they further swear that they have every reason to believe, from the appearance of the applicant and their acquaintance with him that he is the identical person he represents himself to be; and that they have no interest in the prosecution of this claim.
