Sir:

I have the honor to acknowledge the receipt from your Office of application for Pension No. 211,584, and to return it herewith, with such information as is furnished by the files of this Office.

It appears from the Rolls on file in this Office that Balum Bolin was enrolled on the 12" day of Aug, 1862, at Monroe Co in Co. B, 67" Regiment of Ind Volunteers, to serve 3 years or during the war, and mustered into service as a pvt on the 19" day of Aug, 1862, at Madison, Ind, in Co. B, 67" Regiment of Ind Volunteers, to serve 3 years, or during the war. On the Muster Roll of Co. B, of that Regiment, ["for the months of" lined out] from enrollment to Oct 31, 1862, he is reported absent without remark. Return for Sept/62 not on file "The company in engagement at Mumfordville, Ky Sept 14 & 16, taken prisoners of war and paroled on the 17th." Nov & Dec/62, "absent sick at home Monroe Co, Ind, since Nov 26/62, and so reported to Apl 30/63. May & June/63, "discharged, Indianapolis, Ind. May 8/63", for disability.

Cert. of Disability reports him dschd " Indianapolis Ind. May 6th, 1863," which date is accepted as correct.

Sir:

In the case of Balaam Bolin No. 211.534 late a private, Co. "B", 67th Reg't Ind. Vols. it is respectfully requested that you furnish this Office whatever evidence the rolls of said Regiment may afford as to the applicant's service, disability, and discharge.

He alleges he was enlisted on the 12th day of Aug. 1862, disabled by "Chr Diarrhoea & heart disease" contracted in Kentucky Sept., 1862, and discharged on the 6" day of May, 1863, [blank].
