In the invalid claim, No. 211,534, of Balaam Bolin, late a Priv Co. B. 67 Regiment of Ind Vols., it is requested that you will furnish this Office, from data given below, a record of his treatment.

Allegations as to time, place, and circumstances. Disabled by Chronic diarrhea & heart disease contracted in Kentucky, September 1862.
