SURGEON'S CERTIFICATE.

I Hereby Certify, that I have carefully examined Balam Bolen a private of Company /D/ 67th Regiment Indiana Vol and find that Suffering from general Debility resulting from Chronic Diarrhea and that in consequence thereof, he is, in my opinion, incapacitated for duty, and will be during the period of twelve days from this date.

FURLOUGH.

TO ALL WHOM IT MAY CONCERN:

The bearer hereof, Balam Bolen, a Private of Captain Samuel Denny Company B 67 Regiment of Indiana Vol aged 33 years, 5 feet 9 inches high, Dark complexion, Gray eyes, Dark hair, and by profession a Farmer; born in the State of Kentucky and enlisted at Ellettsville in the State of Indiana, on the 13 day of August eighteen hundred and sixty too to serve for the period of 3 years, is hereby permitted to go to Ellettsville in the County of Monroe State of Indiana he having received a Furlough from the Twenty Sixt day of November to the Eight day of December 1862, at which period he will rejoin his Company or Regiment wherever it then may be, or be considered a deserter.

Subsistence has been issued to said Balam Bolin to the Twenty Sixt day of November 1862, and pay to the ["Thirty first" lined out] day of ["November" lined out] 186[blank], both inclusive.

Given under my hand at Camp Morton, this 26th day of November 1862

EXTENSION OF FURLOUGHS:

A certificate of this attending Physician, giving in detail the true condition of the Soldier, the extent to which the wound or disease deprives him of the use of limb or faculty, attested by a certificate of a Magistrate or Notary Public that the Physician giving the certificate is a respectable practitioner, will be required in all cases by the officer who gave the Furlough, before he can extend it.
