OF DISABILITY FOR DISCHARGE.
Private Balaam Bolin of Captain Dennys Company, (B) of the Sixty Seventh Regiment of ["United States" lined out] Inda. Vols was enlisted by Lieut Carlton of the 67th Regiment of Ind Vols at Monroe Co, Ind on the Twelfth day of August 1862, to serve 3 years; he was born in Clay Co in the State of Kentucky is Thirty Six years of age, five feet eight inches high, Dark complexion, Dark eyes, Dark hair, and by occupation when enlisted a Farmer. During the last two months said soldier has been unfit for duty 60 days. States that he has not been an efficient [carat insert] soldier [end insert] since Nov 1862 by reason of Chronic Diarrhoea contracted in the service.
