In the claim of Balaam Bolin, on this 4th day of March, 1876, personally appeared before me, a Justice of the Peace in and for the County and State aforesaid, John Campbell resident Stineville in the County of Monroe and State of Indiana, who, being duly sworn according to law, on oath declare as follows: that he was late a Capt. of Co. B. 67" Ind Vols and Ballaam Bolin was a member of said company and that while in the line and discharge of duty as a soldier at or near a place call Mumfordville Ky in Sept 1862 from exposure contracted chronic Diarrhoea and disease of heart and he further say that his knowledge of the above facts is obtained from the following sources, viz: Personal. And that he has no interest nor concern in this matter.

Subscribed and sworn before me, this 4th day of March, 1876, and I certify that the party whose name appears signed to the foregoing affidavit is the person he represents himself to be, and a good and credible witness, and that the contents of the foregoing affidavit were duly read and fully made known to the affiant, before making oath to the same, and that I have no interest in this matter.

I, Wm F Browning, Clerk of the Circuit Court, within and for the County of Monroe, aforesaid, do hereby certify, that William Staly, Esq., whose certificate ["of acknowledgement" lined out] appears to the Instrument of writing to which this is attached, was, at the time of making said certificate, to-wit: the 4th day of March 1876 an acting Justice of the Peace within and for said County of Monroe, duly elected, commissioned and qualified, and that full faith and credit ought to be given to his official acts, and that the signature purporting to be his is genuine.

IN WITNESS of which, I hereunto affix the Seal of said Court, and subscribe my name, at Bloomington, this 9th day of March 1876.
