DECLARATION OF RECRUIT
I, John B. Black desiring to VOLUNTEER as a soldier in the Army of the United States,
for the term of THREE YEARS, Do Declare, That I an 18 years and _____ months of age; that
I have never been discharged from the United States service on account of disability or by
sentience of a court martial, or by order before the expiration of a term of enlistment;
and I now of no impediment to my serving honestly and faithfully as a soldier for three years.

I, John B. (written over the top R.) Black born in Jefferson in the State of Illinois and
aged 18 years, and by occupation a Farmer DO HEREBY ACKNOWLEDGE to have volunteered this 15
day of March 1864 to serve as a Soldier in the Army of the United States of America, for the
period of THREE YEARS, unless sooner discharged by proper authority: Do also agree to accept
such bounty, pay, rations, and clothing, as are, or may be, established by law for volunteers.
And I John B. Black do solemnly swear, that I will bear true faith and allegiance to the
United States of America, and that I will serve them honestly and faithfully against all their
enemies or opposers whomsoever; and that I will observe and obey the orders of the President
of the United States, and the orders of the officers appointed over me, according to the Rules
and Articles of War.

I CERTIFY, ON HONOR, That I have carefully examined the above-named Volunteer, agreeably
to the General Regulations of the Army, and that, in my opinion, he is free from all bodily
defects and mental infirmity, which would in any way disqualify him from performing the duties
of a soldier.

I CERTIFY, ON HONOR, that I have minutely inspected the Volunteer, _____________,
previously to his enlistment , and that he was entirely sober when enlisted; that, to
the best of my judgment and belief, he is of lawful age; and that, in accepting him as
duly qualified to perform the duties of an able-bodied soldier, I have strictly observed
the Regulations which govern the recruiting service. This soldier has Blue eyes, Black hair,
fair complexion, is about 5 feet 7 inches high.

I certify, on honor, that John R. Black a Private of Captain Wm. D. Ward's Company (B) of
the 60th Regiment of Ill Reg't VOLUNTEERS, of the State of Illinois, born in Jefferson Co.,
State of Illinois, aged 21 years; 5 feet 10 inches high; fair complexion, Blue eyes, Light hair,
and by occupation a Farmer, having joined the company at Jefferson Co., Ills, on the 15 day of
March, 1864, (or was mustered in service as a recruit, by Lt. Carter, at Centralia, Ills., and
enrolled in it at the muster into the service of the United States at Centralia, Ills. on the
Seventeenth day of April, 1864) to serve in the Regiment, for the term of S. Three years: and
having served HONESTLY and FAITHFULLY with his Company in the field to the date of his death is
now entitled to as DISCHARGE by reason of Death Died at Chattanooga Tenn May 28/64
The said John R. Black was last paid by Paymaster Lt. Carter to include the 15 day of
April, 1864 and has pay due him from that time to the present date; he is entitled to pay and
subsistence for TRAVELING to the place of enrollment, and whatever other allowances are authorized
to volunteer soldiers, drafted men, or militia, so discharged. He has received from the United
States CLOTHING amounting to $30 dollars, since the 15 day of March, 1864, when his clothing account
was last settled. He has received from the United States $60 dollars advanced BOUNTY.
There is to be stopped from him, in account of the State of _____________, or other authorities,
for CLOTHING, &c., received on entering service, ____________ dollars; and for other stoppages,
viz:_________________________________________ dollars. He has been furnished with TRANSPORTATION
in kind from the place of his discharge to ___________________; and he has been SUBSISTED for
TRAVELING to his place of enrollment, up to the ________________________, 186_____.
He is indebted to ___________________, SUTLER, ___________ dollars.
He is indebted to __________________, LAUNDRESS, ________ dollars.
