 Birth Certificates

July 21, 2006

VitalChek for Broward County, FL

The below information was placed on this site prior to my adoption of Broward County. VitalCheck provides forms to order Birth Certificates. My experience is that their records and forms are up to date.

I recently checked Broward County's local site and their link to Birth and Death Certificates did not work and sent me to a dead page. Consequently, I cannot verify for you that the below information is still correct.

---------

Posted and updated previously in 2002:
IMPORTANT: Broward County will not send a birth certificate to you, unless (1.) it is your birth certificate and you are 18 or older, or (2) you are the parent or legal guardian of the person in question. You must state your relationship and your signature must be notarized. This rule applies whether you are sending to Jacksonville or to Broward County for the birth certificate. In addition, the request must include full name of person, name of father, maiden name of mother and date of birth. You must include a SASE and money order, certified check, certified bank draft or official business check. A personal check drawn on a Broward County bank need not be certified. Make payable to BROWARD COUNTY HEALTH DEPARTMENT. Broward County birth records date back only to 1915. If you do not know the exact date of birth, do a Social Security Death Index Search, which also gives dates of birth.
If the date cannot be obtained, use the Jacksonville address and request a search. There is a $1.00 per year charge for the search, so give at least approximate year(s) that you want searched. Send the amount you think will be required. They will refund or bill you as needed.
Allow about three weeks for delivery.

 Death Certificates

VitaChek Records
Death Certificates are beneficial as they provide dates & places of birth and death, age at death, parents, residence. The cause of death must be requested and will only be provided to immediate family. Obtaining a death certificate should be the first step in search of an obituary. Be sure to do a Social Security Death Index Search, which also gives dates of death.

Death prior to 1990:

Send a written request for a BROWARD COUNTY death certificate.
Include dec'd full name, date of death, $5 and a SASE to:

 Obituaries
In order to save time and money, it is recommended that FIRST a death certificate be obtained. The full date of death and the fact that the death was filed in Broward County should be verified. Be sure to do a Social Security Death Index Search, which also gives dates of birth and death.

    Death prior to 1986:

A search by hand through newspaper microfilms must be undertaken. A very few volunteers from the Genealogical Society of Broward County will do look-ups. Go to Broward County Query Board and post a query asking for a lookup. Be sure to include all information. It is extremely important that at least an approximate date be available. If date of death is not known, do what is suggested in the paragraph above to obtain the date. These look-ups are very tedious work. The volunteer must go to the Main Library, pay high parking fees and search through reams of microfilmed newspapers to find the obituaries, then search for your particular obituary each time. The more information you provide, the more likely a volunteer will undertake the search.
A donation of $10. is requested:
      Genealogical Society (GSBC)
      P. O. Box 485, Fort Lauderdale, FL 33302

      Death after 1985:

Send a written request for a obituary search of the Sun-Sentinel newspaper. Of course, the obituary must have appeared in the Sun-Sentinel. If the obituary is not found, The Sun-Sentinel Sun Search will return your money. Only a few Palm Beach County and Dade County obituaries are published in this Broward County newspaper. If the deceased was a resident of another county, you would probably do better to check with those counties.
If the obituary is not found, The Sun-Sentinel Sun Search will return your money.
Include dec'd full name, exact date of death, $10 and a SASE to:

      Sun Search: Obituaries
      200 East Las Olas Blvd.
      Ft. Lauderdale, FL. 33301
Requests may be made by phone, call 954-356-4324. Credit cards accepted
