The birthdate on the record seems to have been taken from the headstone: Silas died in 1838 at 88, therefore he was born in 1750. 01/01/ means 'exact month and day not known.'
The notes entry "Wife of Silas Fay" must refer to the fact that Silas' wife is also in the cemetery records. There is a possibility that it was merely misplaced and should have been on Anny's record, since it would be correct there.
