Certificates are $22.00 each, by mail and in person.
Certificates are $29.00 each, by internet or by phone.

NOTE:
Birth Certificate: Individual must have been born in Ohio, an individual can purchase a BIRTH certificate for any county at any health department in any county, in Ohio.

Death Certificate: Individual must have passed away in Auglaize County, this can't be requested from another health department in another county.

REQUEST IN PERSON
- Visit the office: Monday - Friday, from 8:00 a.m. - 4:30 p.m.
- Print out and complete the birth/death certificate application (Please fill out one application for each different person)
- Pay Fee: Cash, check, or money order is accepted.
- Request is processed while you wait.(Agian, this can for BIRTH ONLY this can be done at any Health Department in the state of OHio.)
REQUEST BY MAIL
- Print out and complete the birth/death certificate application (Please fill out one application for each different person).
- They accept money orders or personal checks for payment. If sending a personal check, please put your valid Driver’s License on the check. Make the check payable to Auglaize County Health Department.
- Requests are processed and returned by first-class mail on the following business day.
- Send your request along with the appropriate fee to the following address:

- These orders can be placed during regular business hours.
