Family Group Sheets may be downloaded for free from Ancestry.com.
 Other types of charts may also be found for free on About.com

Most genealogy programs will create a varity of reports including Family Group Sheets

If you do not have a genealogy software program follow these instructions for submitting
 your Family Group Sheets.

The sample below is made for a family of 5 children. To simplify listing the Family Group
 Sheets and preventing duplication, the charts will be listed under the husband's surname.

If there is more than one spouse, please submit a Family Group Sheet for each.
