In your letter requesting information, state your relationship to the patient (if patient is alive, must have patient's written approval), date of birth, name of husband or wife or any and everything you have to help identify the correct patient.

There was a flood is the basement at one time that destroyed some records. It was also suggested that at one time there was a fire.  Don't know about that, but we know that there was a flood.

If your relative  was committed, there are probably lots of Court Records in the county of  residence.  In some counties such information may be found in the Probate Records.
