Remarks: I made an investigation of the accident which caused the death of John Wiles at the Reese Coal Company, Mine No. 2, located about 2 miles west of Panama, Oklahoma. The accident occurred in the fourth panel slope in an old abandoned pump room, the wires were about five feet from the slope in room neck, the slope was about ten feet wide and four feet high. Mr. Wiles place where he was loading was about one hundred twenty-five feet from where the wires are located. There was no occasion for him to g in where the wires were. There was no one seemed to know how the accident happened. Ed Seberam was the man that found him and said he was lying about four feet from the wires. Dr. E. L. Collins of Panama, Oklahoma, said that death was caused from an electric shock.

After a thorough investigation I came to the conclusion that this was an unforeseen accident. Mr. Wiles leaves a wife and child.
