Madison Ann Toliver, infant daughter of Rodney and Peggy Toliver of Sapulpa, died Thursday at Hillcrest Medical Center in Tulsa.

Funeral services are scheduled for 10 am Tuesday at Smith Funeral Chapel with Tommy Toliver officiating.

Burial will follow at South Heights Cemetery.

Miss Toliver was born May 23 at Hillcrest Medical Center.

Survivors include grandparents, James and Shirley Toliver of Sapulpa, and James and Cathy Love of Egypt; and great-grandparents Walcie Toliver of Sapulpa and Mr. and Mrs. James Love of Bixby.
