Samuel Madden, 2 days old, of 6254 S. 97th West Ave., Tulsa, died Friday at St. Francis Hospital in Tulsa.

Services will be 10:30 am Monday at Green Hills Mausoleum Chapel with Pastor Don McIntosh officiating.

Burial will be at Green Hills Memorial Gardens under the direction of Smith Funeral Home.

The Baby was born Wednesday at St. Francis Hospital to Andy and Jackie Madden of Tulsa. 

Other survivors include brother Nicholas; grandparents Rev. and Mrs. Richard Madden and Erma Lee Malone, all of Tulsa; and great-grandmother Bessie Malone of Tulsa.
