Know ye, that Cyrus M. Ward a private of Lt. Thomas E. Smith's Company
(H), 79 Regiment of Ohio Volunteers, who enrolled on the 14th day of Aug.
1862 to serve 3 years or during is hereby
Discharged from the service of the United States, this 16th day of May 1865,
at Camp Dennison, Ohio, by reason of Surg Ccertif of Dis & G.O. 36-65 War
Dept & AGO series of 1862.
Said Private C. M. Ward was born in Waynesville, in the State of Ohio, is 24
years of age, 5 feet 11 inches high, sandy complexion, blue eyes, red hair,
and by occupation when enrolled, a farmer.
Given at Dennison Infa Hospital, this 16th day of May 1865.
