Other information from submitter:  At some point John Cook decided he would go by this name instead of the family name of KOCH. John was the eldest of 3 sons,  also Joseph & William, but was the only one who decided on the COOK spelling.  His parents and brothers are also at St Mary Cemetery.  His death certificate indicates his father was born in Falmouth, Kentucky which I KNOW is wrong. John's grandfather, John Koch is buried in Falmouth. Other family members decided to use the COOK name as well.
