We whose names are hereunto named being Elders and Deacons of the Low Dutch Reformed Church of Schohary Kill and Blenheim as hereby certify that we the Minister Elders and Deacons and our successors in office shall be known and ______ wished by the _______or name of the Low Dutch Reformed Church of Schohary Kill and Blenheim agreeable to an _____past of the 27th March 1801 entitled an act to provide for the incorporation of Religious Societies—Given under our hand and seal this 15th June 1802

Be it remembered that on the Eighteenth Day of June in the year of our Lord One Thousand and eight hundred and two personally appeared before me Samuel Van Vechten, Abraham Hoagland who being sworn said that they said subscribers Tunis Rapelye, Cornelius Lane (?), Lawrence R. Decker, Isaac Hardenburgh, Daniel Cowenhoven and Peter Striker subscribe their names to the within Instrument their voluntary Act and Deed to and for the use and purposes therein maintained and that herein John Hoagland subscribes the same as Witness together with himself and having examined the same & finding no material erasures or  interlinations or obliterations, do allow the same to be Recorded.
