We the subscribers Elders and Deacons of the Protestant Reformed Dutch Church of Leeds and Kisketom being duly assembled  do Certify whomsoever it may concern: that by leave of the Proper Ecclesiastical authority we have assumed the Title of “The Protestant Reformed Dutch Church of Leeds & Kisketom”  and according to the Statute in such case made and provided will be known and distinguished in law, as the body corporate by that title.    In witness whereof we have hereunto set our hand and seals the twenty fifth day of March in the year of our Lord one thousand and Eight hundred & thirty three.

State of New York  Greene County SS  On this twenty sixth day of March Eighteen hundred and thirty three before me the subscribers a commissioner of deed for the county of Greene appeared John C. V. Liew residing the town of Catskill in the county of Greene with whom I am personally acquainted who being by me duly sworn deposes that he resides in the town of Catskill, that he saw John Schuneman, Frederick Sax Junior, James Van Deusen, Joseph Bevier, James Manning, Terah Ferry, David C. Abeel and James Elting  severally execute the within instrument, that he subscribed his name thereto as a witness and that he knew the said John Schuneman, Frederick Sax Junior, James Van Deusen, Joseph Bevier, James Manning, Terah Ferry, David C. Abeel and James Elting to be the persons described in and who executed the said instrument.
