We the subscribers Bethel Judd Rector of the Christs Church of Coxsackie in the County of Green and State of New York and Shadrack Hubbell and David Waterbury Members of the said Church according to the form of the Act to the Legislature of the State of New York passed on the twenty seventh day of March 1801 entitled and Act to provide for the association of Religious Societies do herby Certify that on Tuesday the first day of July in the year of our Lord one thousand eight hundred and Six the Male persons of full age of the church afore said which is in communion of the Protestant Episcopal Church of this State who have been received therein and have attached themselves to the Protestant Episcopal Church, wait according to public notice given in time of morning Service on the Sundays previous thereto according to the said Act for the purpose of incorporating their class under the said Act and by a Majority of voices to Elect two Church Wardens and eight vestrymen and to determine on what day of the Week called Easter week the said officers of Church Wardens and Vestrymen should annually thereafter _____ and their Successors in office be chosen at which Election the said Bethel Judd presided and we do further certify that Reuben Rundell & Nathan Burrows were at said Election elected Church Wardens by a Majority of voices and that Shadrack Hubbell Obediah King David Waterbury  Eliakim Reed Solomon Palmer Stephen Lubdell Samuel Webb & David Everts were elected by a majority of voices vestryman and further that Thursday in Easter week was fixed on as the day when the said officers of Church Wardens and Vestryman shall annually _____ and their successors in Office be chosen And farther that the said Church shall be known in Law by the where of title of the Rector Wardens and Vestry of Christs Church in the town of Coxsackie.

In witness whereof we have hereunto set our names and seals this first day of July in the year of our Lord one thousand eight hundred and Six--- Signed Sealed and delivered in presence of the _____Societies in the _____ _____ interlined before execution—

_____ ___ Be it remembered that on the twenty Six of July in the year of our Lord one thousand eight hundred and Six before me Daniel Sayre one of the Judges of the Court of Common Pleas of the County of Green came Solomon Palmer and the subscribing witnesses to the preceding Certificate to me well known and made oath that he is well acquainted with Bethel Judd Shadrack Hubbell & David Waterbury in the said Certificate described and that he said  the said Bethel Judd Shadrack Hubbel and David Waterbury execute the same freely for the uses therein mentioned and thus the said Solomon Palmer and Timothy Whetmore to him well known severally subscribed their names as Witnesses to the execution thereof and I having examined the same and finding therein so material Erasures as interlinations except the one noticed.
