We the subscribers do hereby certify that agreeable to an Act of Legislature of the State of New York Entitled an Act for the relief of the Protestant Episcopal Church in the State of New York passed the seventeenth day of March in the year one thousand and seven hundred and ninety five after public notice having been duly given the Inhabitants of the Town of Cats Kill County of Green and State of New York proceded to incorporate themselves by the name of the Rector Church wardens and Vestrymen of St. Lukes Church in Cats Kill the Reverend Ammi Rogers A. M. in the Chair when the following persons were duly elected to their respective offices Viz: Thomas Thomson M. D. and Major Samuel Haight Church wardens, Doctor Caleb Benton, Jonathan Boothe, John Andrews, John G.D. (?) Scott, John Blanchard, Frederick Chollette, Solomon Chandler and Isaac Nichols vestryman that Tuesday in the week called Easter Week should forever there after be the day of their annual Election.

In Witness whereof we have hereunto set our hand & seal this twenty fourth day of August in the year of our Lord One thousand and eight hundred an done.

Thomas Thomson, Samuel Haight - Church Wardens

Be it remembered that on the thirty first day of August One thousand eight hundred and one personally came and appeared before me Samuel Van Vechten Esq. One of the Judges of the court of Common Pleas for the county of Green Thomas Thomson and Samuel Haight the church wardens within mentioned who being both to me personally known did acknowledge that they executed the within Instrument for the purpose and uses thus mentioned and I do certify that I am fully satisfied that the said Thomas Thomson and Samuel Haight are the same persons who signed and sealed the said Instrument.

I having examined the same and for no material alterations or interlinations Do allow the same to be Recorded.
