In pursuance of an Act of the Legislature of the State of New York entitled an Act to provide for the Incorporation of Religious Societies passed the 27th day of March 1801 The Ministers of the said society commonly called the Presbyterian Society in the Village of Cats Kill convened at the court house on the 5th day of March 1804 after due legal notification for the purpose of forming themselves into a Society conformable to said Act, where the subscribers were chosen to preside in said meeting and judge of the qualifications of the Electors, who justly certify that the following Persons were duly chosen Trustees by the name of the Trustees of Christs Church of Cats Kill and were classed in the following order

To serve for one year:
Charles Rogers, John Rude (?), Philip Gebhard

To serve for two years:
Stephen Day, Thomas Hale, Nathan Elliot

Richd. Hill, Benjamin Dwight (?), Isaac Dubois to serve for three years.

In witness whereof we ____hereunto set our hands and seals this fifth day of March one thousand eight hundred and four.

Be it remembered that on the sixth day of March in the year of our Lord One thousand and eight hundred and four personally appeared before me Samuel Van Vechten one of the Judges of the court of Common Pleas in and for the county of Green Thos Hale and Thos B. Cook to me known personally who severally acknowledged to have executed the written  Instrument by signing Sealing and delivering the same as their Voluntary Act and Deed to and for the uses and purposes therein mentioned & I do Certify that I am satisfied that the said Thos Hale and said Thos B. Cook are the same persons who executed the said Instrument and I have examined the same and find no material erasure, interlination or obliterations do allow the same to be recorded
