The instructions on this page are for obtaining records of veterans who served in the US military prior to World War 1 ONLY. To obtain records relating to service in World War I or II, or subsequent service, write to:

All requests for veterans records must be submitted on a NATF Form 80. Use a separate form for each file that you request. To obtain NATF Form 80, write to the following address:

Below is an actual NATF Form 80. This form has three carbons, so be sure to press hard when filling it in. The instructions for filling out the form are self-explanatory.
